    AERIS

    Divecomputer

    Aeris has recalled several dive computers. The serial numbers of the affected dive computers are as follows:

    Aeris Savant: Serial #0001 - 1779
    Atmos Pro & Atmos Sport 100S & 300G: Serial nos computers serial #0001 - 5999
    Contact Aeris or call an authorized Aeris dealer to arrange factory servicing.

    EPIC Dive Computer

    Aeris is recalling all EPIC Dive Computers with serial numbers 1 – 712 that were sold between:
     October 1, 2006 – February 7, 2007.
    Aeris has been informed by it’s supplier that they have received isolated reports that two EPIC dive computers locked into the Gas Switch-to screen and did not return to the Main Dive screen when the divers switched the units from Gas 1 to Gas 2 during dives when they were only using one transmitter and Pressure associated with Gas 2 was being provided by an Analog Gauge. You must discontinue all diving activities with this computer until a software upgrade has been accomplished by Aeris

    Elite T3 Dive Computer Safety Upgrade

    All Aeris Elite T3 Dive Computers with serial numbers 1 – 1197 that were sold between:
    August 1, 2006 – July 10, 2007.

    Upon activating the Buddy Pressure Check routine after having previously checked their buddy’s pressure, the previous pressure value was still displayed instead of the message NOT AVAIL which is what should appear on the screen when the buddy is out of signal range when a check is performed.

    Aeris is offering a free operating software Upgrade to all Elite T3 product owners to eliminate any inconvenience during activities involving Buddy Pressure Checks. The Elite T3 unit’s software will be upgraded to revision r1C.

    AT-400 First Stage Regulators

     

    October 23, 2007– San Leandro, CA – AERIS has been notified by its supplier (ROMI, Ent.) that they have received isolated reports of AT400 type Regulator First Stages exhibiting uncontrolled flow of air to the Second Stage.

    Evaluation has shown that a specific batch of HP (High Pressure) Seats contained material that can deteriorate and fail to properly perform its mechanical sealing (shut off) function, causing an uncontrollable free flow which could result in serious injury to or death of the user.

    In cooperation with the U. S. CPSC (Consumer Product Safety Commission), ROMI is initiating a voluntary recall of all affected AT400 First Stages.

    The affected First Stages include those that were shipped to AERIS Retailers from the factory between May 1, 2006 and November 15, 2006. Other Diaphragm type First Stages that received annual service between May 1, 2006, and October 22, 2007, may also be affected.

    NOTE: Due to the random nature utilized to assign SNs (serial numbers) to First Stages, there is only a small quantity of affected units within the large range of SNs. Since the extent of listing the actual SNs of all affected units would be impractical and not user friendly, AERIS has created a data base on the diveaeris.com web site that product owners and retailers can easily access to determine whether a specific First Stage is affected and if it has received the retrofit.

    If you have an affected unit, you must discontinue further use of it and take it to the AERIS Dealer where you purchased it, or a local Authorized AERIS Service Center, for a component retrofit at no charge.

    If you no longer own the Diaphragm type Regulator First Stage, please forward this letter to the new owner or have them contact us directly.

    WARNING: DO NOT CONTINUE USE of the Regulator First Stage until it receives a Retrofit. Only Authorized AERIS Dealers and their trained service technicians must perform this Retrofit. Non authorized persons who attempt the Retrofit procedure risk voiding the product’s warranty and rendering the product dangerously unsafe for SCUBA use.

     

    APEKS

    REGULATOR TX, ATX, XTX

    SEPTEMBER 2008

    These regulators can be missing the diaphragm cover which can cause the diaphragm to become displaced during a dive, allowing water to enter the scuba regulator.
    This recall involves Apeks TX, ATX, and XTX second stage regulators that have never been serviced. Apeks and TX, ATX, or XTX is printed on the regulator.
    February 2000 through June 2008
    Remedy:
    immediately stop diving with these regulators and visit an authorized Apeks dealer for a free inspection and free installation of a diaphragm cover, if the cover is missing.

    WTX BCD

    June 2010

    Aqua Lung has been made aware of a case of a poorly retained oral inflate button that has worked loose from an Apeks WTX inflator. If the button were to come off, the spring behind the button would also come off thereby removing the tension needed to keep the valve closed. If this were to occur in the water, it could lead to a leak of the BC contents out of the oral mouthpiece.

    All Apeks dealers and distributors have been made aware of this issue.

    Do not dive with WTX BCD until it has been inspected by an authorised Apeks dealer. If you require any further information please contact Apeks Customer Service Department.

    “RECALL To all Apeks Customers. There maybe a potential problem with the yoke clamp screw on any yoke clamp type regulator with the serial number starting from 7010001 – 7053528.

    REGULATOR TITAN / CONSHELF DIN

    Titan DIN 1st Stage Scuba Regulators and Titan/Conshelf DIN Scuba Adaptors.
    Over-tightening of the DIN retainer by a technician during installation can result in the retainer breaking under pressure, a rapid escape of air from the scuba cylinder, and the regulator detaching from the scuba cylinder. One report of a DIN retainer breaking under pressure. No injuries have been reported.
    Recalled regulators have a brass DIN retainer mfg'd prior to June 2006. The recalled Aqua Lung Titan Din 1st Stage regulators have serial numbers lower than 6062501 stamped on the side of the regulator’s body. Recalled Titan/Conshelf DIN adaptors are marked “300 BAR MAX” on the side of the part.

    WTX POWER INFLATOR

    JUNE 2010


    Description:The oral inflator button is not properly bonded to the oral stem and can fall off during use, posing a leak of the buoyancy compensator contents.

    Units: About 1,380 in the U.S., 530 in Canada

    Sold at: Diving stores in the U.S. and Canada from November 2006 through March 2010 (as standalone component or as part of a BC)

    The power inflator is the black mouthpiece with the two brass buttons at the end of the corrugated hose. The recall involves all models of the Apeks WTX power inflators. Some of the recalled components were included on complete air cells. Those model numbers are: 388032, 388060, 388080, 388145, 388260, and 42775. The model number of the air cell can be found on the tag sewn on the center of the air cell or bladder.

    This is a voluntary recall of all BCDs manufactured with a serial number between the range of BR0908000 and BR1208999. Specifically, the recall includes the VOODOO, BLACKJACK, BACKWING and CURVE buoyancy compensators.

    BARE was recently contacted by one of their customers who identified an issue with the threaded seal rings that connect the left shoulder rapid exhaust valve and dump valves to the BCD bladder. Under extreme circumstances, when the diver pulls on the inflator hose or release cord to release air from the bladder, the threaded seal ring may crack or break.

     

    Bare - SI TECH Drysuit Inflation Hoses

    Bare is recalling all SI TECH Drysuit inflation hoses that were sold between: June 1, 2006 - February 29, 2009.

    Bare has been informed by it's supplier that they have received isolated reports that the flow restricting insert may malfunction by dislodgment from the connection valve, potentially causing an obstruction of air flow. You must discontinue all diving activities with this SI TECH inflation hose until the removal of the flow restrictor has been accomplished

     

    CRESSI

    REGULATOR ELLIPSE BLACK MC5

    December 2009

    Cressi USA has announced a safety recall of its Cressi Ellipse Black MC5 Scuba Regulator. This recall affects Ellipse Black MC5 regulators distributed and sold from March 2009 through August 2009 are affected by this product recall.

    The words “Cressi Black” appear on the cover of the regulator’s second stage, and the words “Cressi MC5” are visible on the regulator’s first stage.  (See photo above.)   Retail price ranges between $280 and $309.

    Hazard: Partial obstruction of the High Pressure port can produce an inaccurate reading on the pressure gauge, resulting in a slow descent of the needle in the pressure gauge.  The inaccurate reading on the gauge poses a drowning hazard to divers.

    DSS has received a limited number of reports of leaking Low Pressure Hoses. These are the Grey hoses furnished with DSS wings for the purpose of connecting the power inflator to the first stage regulator. The leaks occur at the crimped end fittings.

    Any hose purchased between January 1, 2010 and June 30, 2010 should be removed from service.

    DIVE RITE

    December 2009

    The U.S. Consumer Product Safety Commission, in cooperation with Dive Rite, has announced a voluntary recall of v. Partial obstruction of various Wings.

    The over pressure valve springs in the diving equipment could rust and fail allowing the buoyancy compensator devices to leak, posing a drowning hazard to divers. No injuries have been reported.

    Springs were used on wings that have an opaque white or blue tinted bladder and welded in flanges. Wings with a black bladder are not affected. Only the Dive Rite wings that have a serial number range from 42000 through 72000 are affected by this recall. The serial numbers can be found on a white tag attached along a seam of the wing.

    Consumers should immediately stop using recalled diving equipment and return it to an authorized distributor or dealer for a free replacement of the overpressure valve spring.

    Halcyon Manufacturing Inc. in cooperation with the US Consumer Product Safety Commission has initiated a voluntary recall of select Eclipse, Evolve, Explorer, Pioneer and CCR35 buoyancy compensators (BCDs) manufactured between January, 2006 and September, 2008.

    In addition, select Halcyon inflatable devices (Lift Bags, SMB’s, DAM’s, Surf Shuttles and Diver Lift rafts) may also be affected.

    These BCDs or Inflatable Devices could develop excessive corrosion or rusting problems with the stainless steel Over Pressure Valve (OPV) spring, causing the spring to fail and the BCD or Inflatable Device to leak through the OPV.  It is unlikely that most of these units will experience any problems since the potential defect is limited to a very small portion of the total number of OPV springs used during this time period. However, given the potentially serious implications all OPV springs should be checked to minimize risk or inconvenience to Halcyon customers.

    BCDs manufactured after September, 2008 are not included in this voluntary recall. Please reference press release and website information for details on how to identify the BCD serial number. Inflatable Devices do not have serial numbers and should be checked based on customer purchase date if possible. If there is any doubt as to the serial number or date of purchase, Halcyon recommends inspecting the OPV spring at the request of the customer.

    COMPUTER NEMO AIR - 2nd RECALL

    APRIL 2010

    Mares Diving has announced a product safety recall involving the company's NEMO Air scuba diving computer. Under certain circumstances, the O-ring can fail and cause a slow leak of breathing gas through the Quick Connector, which could require a diver to surface quickly and face possible risk of injury or running out of air.

    Any air leak resulting from an O-ring failure would be slow, because these fittings are designed to the EN250 standard, which requires a very small (0.3mm) opening in the high-pressure hose fitting, but we nevertheless take this issue seriously and intend to correct all affected units.

    The O-rings in some units may have been replaced in an earlier service campaign, but this recall requires replacing the entire metal quick connector female fitting at the end of the high pressure air hose that holds the O-ring (new Mares part no. 44200829). Replacement connector assemblies have a groove machined around the middle of the fitting, but recalled units do not.

    ALL CONSUMERS SHOULD STOP USING ANY NEMO AIR DIVE COMPUTER, AND ALL RETAILERS SHOULD REMOVE THESE UNITS FROM DISTRIBUTION, UNTIL THEY HAVE BEEN INSPECTED AND REPAIRED BY AN AUTHORIZED MARES DEALER / SERVICE CENTER.

    COMPUTER NEMO AIR - 1st  RECALL

    MARES Diving issued the following announcement earlier today:

    MARES DIVING URGENT SAFETY NOTICE NEMO AIR QUICK CONNECTOR 0-RING

    Please be informed that we have discovered a quality issue on the O-ring assembled on the Nemo Air quick connector.

    Under certain circumstances, the O-ring can fail and consequently Nemo Air may start leaking through the Quick Connector. As a result, there is a continuous, albeit slow, loss of breathing gas and Mares has decided to issue a voluntary product recall. After intense internal tests Mares has identified a new O-ring of different material, color and hardness that can be retrofitted onto the Quick Connector of any existing Nemo Air.

    We take this matter very seriously, but we would also like to point out that this issue is covered by the EN250 norm, which requires a 0.3mm opening in the high- pressure hose fitting. As a result, the hose is designed to ensure a slow enough gas release; which should allow the diver to complete a safe emergency ascent.

    NONETHELESS FOR THE PERSONAL SAFETY OF YOUR CUSTOMERS AND TO AVOID POSSIBLE DANGEROUS ACCIDENTS: STOP ANYONE FROM DIVING WITH A QUICK CONNECTOR SUBJECT TO THIS VOLUNTARY RECALL UNTIL THE EXISTING O-RING IS REMOVED AND SUBSTITUTED WITH THE NEW GREEN COLOR ONE.

    WE HAVE BEEN INFORMED THAT SITECH (The manufacturer of) DRY SUIT INFLATION HOSES SUPPLIED TO MOBBY’s, THE PRODUCER OF MARES DRYSUITS ANNOUNCED A RECALL ON THEIR LOW PRESSURE INFLATION HOSES. THESE HOSES ARE SUBJECT TO A RECALL FOR THE REMOVAL OF THE PLASTIC FLOW RESTRICTING INSERT!

     
    All end consumers/owners must send the inflator hose back to the Mares Dealer where the suit was purchased or to the MARES US SERVICE CENTER so that the insert can be removed at no charge.
     
    We encourage everybody who is reading this SAFETY NOTICE, to notify all end users immediately so they can receive this information in a timely manner, as this is a safety concern.
     
    The low pressure hose contains a flow restricting insert, which can be of black, blue or green color. Reports of an indicated malfunction of the flow restricting insert in the hose for the suit have been submitted to SITECH. The insert may malfunction by dislodgement from the connection valve, potentially causing an obstruction of the air flow.
     
    THEREFORE FOR YOUR PERSONAL SAFETY AND TO AVOID POSSIBLE DANGEROUS ACCIDENTS:

    An internal component that seals air between the high pressure first-stage and the intermediate pressure second-stage can fail. This can result in uncontrolled flow of air to the diver and pose a risk of serious injury or death.
    ROMI has received two reports of units malfunctioning during diving and six reports of units malfunctioning while being tested by dealers prior to delivery to consumers. No injuries reported.

    Oceanic - OC1 Firmware Update

    March 2, 2010

    Important 1C Firmware Update from Oceanic.

    Units affected are the OC1 model containing version 1A or 1B firmware.

    In response to input received from those who have now experienced ownership and use of the OC1, Oceanic has corrected or modified the following items to improve the OC1's operation:

    *** Eliminated premature entry into Decompression mode upon descending on NORM Nitrox dives.  This occurred when the dives were conducted after setting FO2 Gas 1 for Nitrox (a numerical value) followed by the M button being pressed to step back through the Plan Mode's Pre Dive Planning.

    *** Corrected Temperature displays that appeared only as 0 degrees on the Dive and Log mode screens when the associated dive was activated by the wet contacts upon descent while in Watch mode.  Temperature displayed as expected during other conditions.

    *** Inserted the far left digit of the Watch Countdown Timer which remained blank with Hour Format set for 24 Hour.

    *** Modified the Set Watch Daily Alarm and Countdown Timer, and Free Mode Countdown Timer, functions to make them to improve ease of access and saving of the selection.

    BC Sealing Rings BCA-500

    Description: Molding variations in this part as used in the OMS Buoyancy Control System (BCS) could cause cracking or breakage of part, resulting in rapid loss of buoyancy, creating a potential drowning hazard.

    Incidents/Injuries: There have been reported of 19 failures. These failures were discovered during inspection, with no reports of injuries or death.
    There are a total of 19,790 BCS Seal Rings (Item # BCA 500) installed in 5730 BCS mfg'd from May 11, 2006 through August 1 2007.

    While this variation is not present in every Seal Ring (Item # BCA 500), it would be difficult to determine in the field which Seal Rings are suspect. Therefore we are requiring that all Seal Rings be updated with new assemblies. All NEW seal rings are equipped with reinforcement at critical areas.

    Full Face Mask - with serial numbers 9051284 through 100070954 that were sold from September 2010 through November 2010

    OTS is voluntarily recalling all Guardian Full Face Masks with serial numbers 9051284 through 100070954 that were sold from September 2010 through November 2010.

    OTS has received isolated reports that the Guardian visor has dislodged from the mask causing your mask to unsafely flood. Consumers should immediately stop using the diving masks and contact Ocean Technology Systems for instructions on conducting a test of the regulator and returning the masks for a free repair.

    OTS has received isolated reports that the Guardian visor has dislodged from the mask causing your mask to unsafely flood. You must discontinue all diving activities with your OTS Guardian Full Face Mask until getting retrofitted visor claims by sending your Full Face Mask into OTS.

    Besea W50 Diving Wings with Poseidon Inner Bladders

    The inner bladder located inside the diving wing can break, causing the wing to fail to operate as a floating/buoyancy device.

    Poseidon has received 15 reports of inner bladders breaking. No injuries have been reported.
    Description: The recall involves the Poseidon inner bladders with batch number 5445 sold with the Poseidon Besea W50 wings. The inner bladder is located inside the outer cover of the wing. The batch number and “Poseidon” are molded on a tab located between the “legs” of the inner bladder.

    Sport Custom Mouthpiece

    SeaCure is recalling all Sport I, Sport II, and Sport III Mouthpieces that are being used on Atomic regulators (2005- present), Zeagle regulators (all models/years) or Cressi-Sub Ellipse regulators (all years). The SeaCure Sport Custom Mouthpieces with yellow inserts may be clear, blue or gray in color.

    The SeaCure Sport Custom Mouthpieces with yellow inserts were discontinued in 2006 and replaced by the SeaCure Hi-Flow Custom Mouthpieces.  At the time, it was SeaCure’s information that very few of the Sport units were left in retailer’s inventory.  However, SeaCure recently learned of three incidents involving two Sport models that were affixed to an Atomic regulator that was manufactured in 2005 or thereafter and to a Zeagle regulator. 

    Investigation of these incidents revealed a sizing problem with certain Sport custom mouthpieces and the Atomic, Zeagle and Cressi-sub Ellipse regulators.  In order to place the Sport mouthpieces with yellow inserts on these regulators, they have to be stretched to the point that the yellow insert could detach from the wall of the mouthpiece and move.  This has the potential to cause a problem for the diver because the movement of the insert into the second stage of the regulator could make breathing difficult.  If the mouthpiece does not contain a yellow insert, this is not a potential problem because you have the updated SeaCure Hi-Flow mouthpieces.

    Stop using these SeaCure Sport custom mouthpieces immediately if you have an Atomic regulator (2005- present), Zeagle regulator (all models/years) or Cressi-Sub Ellipse regulator (all years).

    Through technical and quality control tests, Seac Sub has identified a potential defect in the removable weight pocket system in the Mito and Muse stabilizer jackets that, in particular situations, may cause the unexpected release of the removable weight pocket.

    In order to guarantee our customers and divers maximum safety during diving and the best possible service, Seac Sub has decided to verify and, if necessary, revise all the stabilizer jackets of these models on the market, as no specific lot number can be identified. The company therefore requests that owners of the products in question stop using them and to bring them to a Seac Sub dealer authorised to provide technical assistance in order to have them undergo the necessary revision and maintenance procedures free of charge.

    PRO 2000 ADV BCD

    Seac Sub releases a security notice relative to the "Air Control" buoyancy system of the "PRO 2000 ADV" BCD up to today out of production.
    By means of technical tests and quality controls, Seac Sub has identified a production batch of the "PRO 2000 ADV" bcd with a defect in the galvanic treatment of one of its components, such to determine a possible bad functioning of the "Air Control" buoyancy system in case poor or unsuitable maintenance.

    In order to safeguard the greatest security during the immersion and to always guarantee the best possible service to its customers and to its divers, Seac Sub decided to examine and eventually service all the BCDs of this model that are on the market, as it is not possible to identify the concerned production batch. Thus the company require to the owners of this product to stop using it and to take it to a point of sale authorized to supply the technical assistance for the Seac Sub products, to obtain the necessary service and maintenance free of charge.

    DRY SUIT INFLATOR HOSES

    OCTOBER 2009

    Description: The hose contains an insert that can dislodge during diving and restrict air flow to the diver, posing a drowning hazard. Incidents/Injuries: SI Tech has received six reports of hose inserts dislodging, including one that was involved in the death of a diver in Los Angeles, Calif.

    This recall involves a dry suit inflation hose that connects a diver’s dry suit to the air supply and allows for the pumping of air into the suit to set up a positive pressure arrangement to help keep it watertight. The hose contains an air flow restricting insert that may be either black, blue or green in color. The batch code is stamped on the threaded metal end of the hose. They were sold with dry suits and also sold separately.

    EXPLANATION

    In response to input received from owners and users of the IQ-900 ZEN, TUSA has corrected or modified the following items to improve the IQ-900 ZEN's operation:

    • Corrected premature auto reset of F01 from a Nitrox setting to an Air setting. This occurred without user action when Watch Mode was temporarily accessed after F02 was set for Nitrox.
    • Changed the No Deco Safety Stop Depth from 4.5M to 5M (metric units). This was requested to be in agreement with local training guidelines.

    • Added EEPROM (memory) protection in the operating code to prevent data stored in the viewable Log from changing. This occurred after conducting NORM dives when data was first displayed in the Log as No Deco then upon access later it was displayed as being Deco and GAUG.
